Insurance Release

To release your vehicle to insurance the following MUST be submitted to our office:

1. Copy of Current Registration (CAN NOT ACCEPT EXPIRED REGISTRATION)

OR

Copy of Vehicle Title/Pink Slip

2. Copy of VALID US Government Issued Photo ID

Examples: Valid state issued Driver’s License, Valid State issued ID card, United States Government Passport, US Military ID, US Gov’t Permanent Resident Card.

Unacceptable examples: Company badges, Costco Card, Social Security Card, IDs issued by another country, or student IDs

Power of Attorney

If you are unable to come in person to retrieve personal belongings OR to pick up your vehicle, the following can be submitted to allow someone on your behalf to do so. This does not apply to any vehicle needing a vehicle release form from the impounding police agency. You must contact them to find out their policies and procedures. Power of attorney can not be submitted via email since the form is notarized.

3. Power of Attorney Form (provided her to download) and this form MUST be notarized




Submitting Forms:

In person:  Monday-Friday, 8am to 5pm during normal business hours, excluding holidays. If you do not have access to a printer or copier, you can come into the office where we can make copies of these items for you and provide the form to be filled out.

Electronically (Insurance Release ONLY): All 3 items can be submitted via email to manager@patriottowca.com. This email is only monitored during normal business hours. All items will be reviewed and if anything does not meet the requirements, an email will be sent back with any issues that will need to be resolved or a confirmation email that everything was received and good to go.

*By submitting these forms, you understand it does not waive financial responsibility. It is your responsibility to ensure your vehicle is picked up by your insurance company or by your Power of Attorney. We do not contact your insurance company for you. In the event, the vehicle is not picked up, the vehicle will be sold at auction in accordance with California state law. Any remaining balance owed, will be submitted to collections. PLEASE do not let it go that far. We would much rather answer your questions on available options.
